Incidents are being investigated with the aim to learn and to prevent they will re-occur. The level of investigation is depending on the severity of the incident. Currently there is no uniform methodology how we can learn in the most effective way of incidents.
The project involves the following aspects:
- collect and analyze all methodologies used in Europe used for incident investigation
- collect all methodologies used in Europe how to learn from incidents, including communication protocols
- literature study on most effective ways how to learn from incidents
- recommendations for improvement
